    Question Passport for married woman who wants to use her husband's surname HELP!

    Hello again people,

    As you may know I got rejected from my visit visa application. Now my husband and I want to visit Cyprus instead but we want to change my passport name to my husband's surname.

    Here's the question. Since I don't have any IDs yet with my husband's surname on it. Will the DFA accept IDs with my Maiden Surname on it instead of my husband's? Or should I apply for new IDs using my husband's surname before I apply for passport? We already have NSO Marriage Certificate. I don't wanna book appointment yet and find out I have to produce IDs with my husband's surname!

    Anyone else has experience about this?

    Hi Anvee.
    I would like to assume that you are currently in the Philippines. For passport application, you need to provide the NSO copy of your marriage contract and CFO sticker (+guidance certificate) since you are married to a foreigner. Other requirements like photo id, application form, fee, etc is available online.

    Hi, I was at the DFA yesterday and they only took my old passport photocopy, CFO certificate photocopy and original copy of NSO marriage certificate.

    My wife managed to change her surname just on the strength of the NSO and marriage certs, that's it.....CFO wasn't required for the name change....this took place in Manila July 2009

    yes now they are being fussy about it because they assume if you change your last name to your foreign husband's name, you're gonna have to migrate abroad which is pathetic! they assume all filipinos want to settle outside the Philippines!!!!!

    Your Old passport, Marriage Cert NSO or CTC in Security paper authenticated by NSO and CFO certification.

    for the CFO be there ealry in the morning cos they only accepts 15 participants.
