She is the only one that needs to fill in a form. Your only input is providing information about yourself, financial/accomodation documentation, any evidence of your relationship, and probably a sponsorship letter (your chance to provide information not included elsewhere).
In regards to the question about when did you first meet, I'm not sure if there is a right answer, try reading this thread.

Steve.
There are a lot of parts on that from that relate to you and your circumstances. You should also provide a cover letter outlining your circumstances (finances, accomodation etc), and a LOT of documentation to prove this. Proof of relationship, proof of finances (bank statements, wage slips etc) proof of accomodati0on, passport and birth certificate details etc etc etc.
All of this is covered in more detail on Petes site (already referenced above), although some of the forms listed on there are a little out of date so make sure you get the current ones.
The Government websites are the best ones to look to find the requirements. People here will help with any confusion I'm sure, but they will expect you to do the ground work yourself.

Its the correct form for a 6 month family visit visa.
If you require your wife to settle in the Uk you require a vaf 4 application form here is some useful links.
