passport ammendment requirements pls. help!!!

I just got my CFO sticker last friday. SO my next step now is to ammend my passport. What are the other requirements? I've read some thread here about it before and added it on my favorites but had to reformat my laptop so I lost everything. I'm searching but just can't find it... Pls help!!!

for the safe side: bring your
NSO birthcertificate

some may ask NBI etc... hay hehe, i think it is case to case basis... some of the centres are quite fussy and some or pretty straight forward.

just have everything handy so you are ready!

I renewed my passport this Sept,and they just asked for photocopies of my Guidance and Counseling Certificate & Certificate of Attendance, both from CFO. And of course the MC from NSO.

THANKS EVERYONE! I went to DFA today but I was so disappointed because I have to go back there again tomorrow to process it. They just stapled a piece of paper on my application which is the schedule ,time and date when you're going to return.

That's how it is nowadays for your new passport.you have to go back for the appearance.

You will be advised by DFA when to go back for personal appearance. The passport amendment usually takes 10 working days of processing. That's how long I've waited when I ammended my passport last April 2008. I was just asked to submit my NSO MC.
